This new installment follows a new group of teens, played by Madelyn Cline, Chase Sui Wonders, Jonah Hauer-King, Tyriq Withers, and Sarah Pidgeon, who find themselves targeted by a mysterious stalker who knows a dark secret from their past. Desperate for help, the group turns to Julie and Ray, the only survivors of the infamous 1997 Southport Massacre. Fans of the original franchise may have expected the return of another key character from the first film: Helen Shivers, played by Sarah Michelle Gellar. However, despite the director's best efforts to bring her back, Gellar will not be appearing in the film.

The reason for Gellar's absence from the new film is rooted in her character's fate in the original movie. Helen Shivers, the character Gellar portrayed, met a gruesome end at the hands of the mysterious killer. Gellar has consistently said she would not return to the franchise, despite her close friendship with Do Revenge director Jennifer Kaytin Robinson and her long-standing relationship with Freddie Prinze Jr., whom she has been married to since 2002.

In an interview with Entertainment Weekly, Robinson opened up about her attempts to persuade Gellar to reprise her role, jokingly admitting that she “harassed” the actress in hopes of convincing her to return. “I tried, okay? I harassed her! But she is dead,” Robinson said with a laugh. Robinson continued with the playful banter, suggesting, “Yeah, but what if?” to which Gellar firmly responded, “I am dead. I am Sarah Dead Gellar.”

In February 2023, during her acceptance speech for the SCAD TVFest Icon Award in Atlanta, Gellar humorously referenced the director's persistent attempts to bring her back. “As I explained to Jennifer, I am dead. I am dead dead. On ice,” Gellar said, reiterating her character’s definitive demise. She joked about her character’s fate, saying, “It’s soap dish dead. I don’t have a head. You can’t write for someone that doesn’t have a head. I’m dead.”

Though Gellar will not be physically appearing in the film, she did reveal in an interview in October 2024 that she has still found a way to contribute to the I Know What You Did Last Summer franchise in an unofficial capacity. Gellar shared that her best friend, director Jennifer Kaytin Robinson, has assigned her a new role on set: continuity. "So I’m always the one telling her, ‘Well, that would happen, or that wouldn’t happen with those characters,’ so I do have kind of an unofficial job title," Gellar explained, giving fans a glimpse into how she’s involved with the film behind the scenes.

While Gellar’s role may be more limited this time around, her connection to the I Know What You Did Last Summer franchise remains strong. The new film will see a subtle nod to her character in the form of a photo of Helen resting on her gravestone, briefly featured in the trailer.
